What services do you need?

Choose an exact option in each service category. In-home services can be combined; GLP-1 and NAD+ telehealth consultations must each be booked separately.

GLP-1 Weight Management
Telehealth only: video evaluation for prescription weight management treatment
Cash-Pay Labs & Collection
Wellness panels, individual tests, or collection for an outside provider order

Wellness panels include collection. Individual tests add a separate $75.00 mobile collection fee.

Vitamin Injection
Fast intramuscular B12 or vitamin D3 injection
NAD+ Treatment
Telehealth only: $75 video consultation and prescription review; prescribed treatment is $150/month for one vial

The $75 consultation includes a health history and goals review, discussion of NAD+ considerations and risks, and clinician assessment. It is not an injection appointment or a guaranteed prescription. If approved, one vial is $150/month. Start prescribed treatment on the consultation day for $50 off the first month's vial only; the consultation is not discounted. Staff arranges either automatic monthly billing or separate payment each month.
